Job Description :

We are seeking an experienced Home Health Registered Nurse (RN) for one of our client which is Hospital,

• Pay Rate: $70/hr
• Job Location: St Johnsbury, VT 05819
• Shift: Days, M-F, possible weekend shifts
• Duration of Contract: 13 weeks
• Guaranteed hours: 36Hrs

Submissions Requirements:

• Current active Vermont RN license required
• Minimum one year of clinical nursing experience preferred
• Home Health experience is highly desirable
• Current BLS certification required.
• Current ACLS certification required.

Job Duties in Order of Importance:

• Conduct patient admissions and comprehensive nursing assessments
• Develop and implement individualized care plans
• Provide skilled nursing care in patients' homes
• Monitor patient progress and modify care plans as needed
• Collaborate with interdisciplinary healthcare teams
