Looking for Hospice Admission Visit RN one of our client which is Home health Care,

• Pay: $58/Hr
• Job Location: Sunnyvale CA 94085
• Shift: Sunday - Thursday Hours: 10am - 7pm or 11am - 8pm
• Contract Length: 13 weeks
• Guaranteed Hours per week: 40

Qualifications:

• Active California RN license
• Minimum 1 year of hospice nursing experience
• BLS/BCLS certification
• Ability to provide hospice admission visits and complete comprehensive patient/family assessments
• Experience developing individualized plans of care
• Ability to coordinate care with a multidisciplinary hospice team
• Ability to document patient assessments and interventions
• Ability to travel throughout the assigned territory: San Francisco, San Mateo, and Santa Clara Counties
• Homecare Homebase (HCHB) experience is the EMR used.

Responsibilities:

• Admit patients to hospice services
• Assess physical, psychosocial, environmental, safety, and developmental needs
• Develop individualized plans of care
• Document assessments and interventions
• Coordinate care with the multidisciplinary team
