.Urgent need for RN ,Care Coordinator for one of our client which is a Medical Center,

• Pay rate: $68/hr
• Job Location: Chico, CA 95926
• Shift: 8-hour shifts, 8am - 4:30pm
• Duration of Contract: 13Weeks
• Guaranteed Hours:40 (weekends and weekdays)

Position Requirements:

• Registered Nurse (RN) with a current, active California RN license.
• At least 1 year of recent Case Management experience in an acute-care hospital setting.
• At least 1 year of nursing experience.
• Current CPR/BLS/BCLS certification.
• Experience or knowledge of care coordination and utilization management.
• Ability to work with Epic EMR is relevant to the position.

Responsibilities

• Coordinate care for assigned patients throughout their entire episode of illness or treatment.
• Assess patients and develop, implement, and evaluate care plans.
• Collaborate with patients, families, physicians, nurses, multidisciplinary teams, and payors.
• Coordinate care for patients in the Emergency Department when assigned.
• Monitor patient progress and coordinate services to support efficient, appropriate care.
• Advocate for patients and families throughout the care episode.
