Urgent need for a RN Case Manager  for one of our client which is a Hospital

• Job Location: 273 County Rd, New London, NH 03257
• Shift: Days(M-F)(8hr)
• Duration of Contract: 13 weeks
• Guaranteed hours: 40 hours per week
• Minimum years of experience: 2 years

Pay rate: $65/hr

Job Description:

• Completes assessment of patient needs and resources within 24-48 hours of admission.
• Evaluates the allocation of services to ensure appropriate resource utilization.
• Serves as a resource to the healthcare team, communicating with physician, consultants, and all the members of the healthcare team.
• Collaborates actively and functions in a multidisciplinary department with the social worker, transitional care nurse, and the inpatient treatment team, to provide comprehensive full spectrum care coordination and discharge planning for each patient.
• Completes concurrent review of medical charts utilizing MCG, to validate the appropriate utilization of resources in patient care.
• Secures authorization for needed additional patient services, as required.
• Coordinates family and patient meetings in order to develop a comprehensive plan of care across the continuum of care delivery.

SUBMISSION REQUIREMENTS:

• Minimum 2 years current experience in a CRITICAL ACCESS HOSPITAL setting - Required
• BSN - HIGHLY PREFERRED
• MCG experience - Preferred
• Prior travel experience - Preferred
• BLS (AHA)
• NH state license required
