**Quick Start**

Urgent need for a Registered Nurse for one of our client which is a Home Care Health.

• Job Location: 24301 Southland Dr Suite 401 Hayward, CA 94545
• Shift: Days
• Duration of Contract: 13 weeks + Extendable
• Shift Notes: 40 hours scheduled/Must be able to work a Tuesday-Saturday or Wednesday-Sunday schedule
• Minimum years of experience: 2 year

Pay rate: $75/hr

Job Description:

Patient Care

• 1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• 2. Regularly re-evaluates patient nursing needs.
• 3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
• 4. Uses health assessment data to determine nursing diagnosis.

Communication

• 1. Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
• 2. Communicates with community health-related persons to coordinate the care plan.

Additional Duties

• 1. Participates in on-call duties as defined by the on-call policy.
• 2. Ensures that arrangements for equipment and other necessary items and services are available.

.SUBMISSION REQUIREMENTS

• 1 year of Home Heath experience - REQUIRED
• Graduate of an accredited school of nursing with 1 year of recent acute care experience - REQUIRED
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - REQUIRED
• Good typing skills - REQUIRED
• Wound care, wound vac, catheter, acute care experience - REQUIRED
• Doesn't need CA license at time of submittal but will have to be willing and able to obtain
• CPR required
• Covid Vaccination required
• Must have CA State License
