• Shift: Varied
• No of weeks: 13
• Guaranteed hours:40 hrs
• Shift Notes: Weekend On Call , Friday 4:30p-8am Monday, traveling within 35 miles of facility, Will be paid 40 hours for call period regardless of calls, 1st week M-F for training, 2nd weeks 0000 Sunday morning finish 08000 Monday then start 4:30pm Friday

Pay rate: $60/hr

Requirements:

• 2 year outpatient Hospice experience, 2 year med surg experience
• Previous primary on call experience needed
• Previous travel preferred
• BLS required
• Active IL State License required
• IL RN License Must be in hand at upload

Job Description:

• Collaborating on terminally ill patients' care with physicians, social workers, chaplains, nursing assistants, and other stakeholders.
• Preparing and adjusting patient care plans as needed.
• Frequently visiting patients to monitor vital signs and physical comfort levels.
• Performing prescribed treatments and administering pain management medications.
• Informing physicians about changes in patients' conditions.
