Job Description :

Urgent need for a Therapy/Rehabilitation with our client, which is a Home Health,

• Job Location: Sacramento, CA 95834
• Specialty: PT Home Health
• Shift: Day ,(08:30 - 17:00) Friday- Monday
• Duration of Contract: 13 weeks
• Guaranteed hours: 40 hrs
• Minimum years of experience: 2 years

Pay rate: $60/hr

Job Duties in Order of Importance:

• Assessing the rehabilitation needs of their patients
• Creating treatment plans and programs that are specific to each patient
• Analyzing their patients progress
• Keeping up to date documents on the patients progress
• Engaging patients in the therapy

Skills Required:

• CA license in hand
• 1 year Home Health experience required
• Oasis experience preferred
• HCHB experience preferred but will train.
• COVID Vaccination required + Booster
