Urgent need for a Social Worker -LMSW for one of our client which is a Hospital.

• Job Location: Maple Grove, MN 55369
• Shift: Days(8:00 AM - 4:30 PM)
• Duration of Contract: 13 weeks + Extendable
• Weekend Requirements: Every 3rd Weekend
• Guaranteed Hours: 40hrs/week
• Minimum years of experience: 2 years

Pay rate: $53/hr

Job Description:

• Assisting clients in receiving services by telephone and in person.
• Getting clients involved in beneficial activities.
• Assessing clients and gathering relevant information.
• Offering information and supporting clients and their families.
• Contacting and making referrals to other agencies and services.
• Maintaining accurate records and preparing reports.

Position Requirements:

• 2+ Years Hospital Social Working Experience - Required
• Acute Care (Hospital) Discharge Experience- Required
• Adult and Geriatric Experience - Required
• BSW, CSW, or MSW - Required
• BLS - Required
• Must have Social Worker License in MN
• Must have MN State License.
