Urgent need for a Therapy/Rehabilitation with our client, which is a Homecare

• Job Location: 17985 US Highway 64 Somerville, TN 38068
• Specialty: PT Home Health
• Shift: Day ,(08:00 - 16:00)
• Duration of Contract: 13 weeks
• Guaranteed hours: 40 hrs
• Minimum years of experience: 2 years

Pay rate: $56/hr

Job Duties in Order of Importance:

• Assessing the rehabilitation needs of their patients
• Creating treatment plans and programs that are specific to each patient
• Analyzing their patients progress
• Keeping up to date documents on the patients progress
• Engaging patients in the therapy

Skills Required:

• PT Home Health experience, 1 year
• Homecare Homebase Charting required
• TN - PT License in hand
• COVID Vaccination Status required / Booster Status required
