We are seeking experienced Licensed Practical Nurses (LPNs) – Hospice for one of our clients, a leading home healthcare service provider.

• Pay Rate: $45/hr
• Job Location: White River Junction, VT 05001
• Shift : Day
• Number of Weeks: 13
• Guaranteed Hours: 40 hours/week

Position Requirements:

• Current and active LPN license in Vermont and New Hampshire, or an active compact license.
• Graduation from an accredited/approved Practical Nursing program.
• Minimum 2 years of nursing experience.
• Experience in hospice, home health, medical-surgical nursing, or a related setting preferred.
• Current BLS/BCLS certification required.

Responsibilities:

• Provide compassionate nursing care to hospice patients in their homes.
• Make home visits under the direction of the RN Case Manager.
• Monitor patients' physical and emotional condition and report changes promptly.
• Assist with end-of-life care and comfort measures.
• Administer medications and treatments as directed by the care plan.
• Educate patients and families regarding care, medications, and symptom management.
