**Quick Start**

We are seeking an experienced Licensed Practical Nurse (LPN) to join a Extended Care/Skilled Nursing Unit (SCU),

• Pay Rate: $40/hr
• Job Location: Kremmling, CO 80459
• Shift: Night Shifts - Variable Days 12 hour shifts
• Duration of Contract: 13 weeks
• Guaranteed hours: 32hr guarantee

Position Requirements:

• Active Colorado LPN license (required)
• Minimum 2 years of experience (required)
• Current BLS (Basic Life Support) certification
• Experience in Med-Surg, Long-Term Care (LTC), and/or Special Care Unit (SCU).

Responsibilities:

• Provide direct patient care in the Extended Care/Skilled Nursing Unit.
• Assess, plan, implement, and coordinate individualized patient care.
• Monitor patient conditions and report changes to the healthcare team.
• Administer medications and treatments as prescribed.
• Maintain accurate patient documentation using EPIC EMR.
• Collaborate with physicians, nurses, and interdisciplinary teams.
