Job Description :

We are seeking an experienced Home Health/Hospice RN to join our Nursing home.

• Pay Rate: $54/hr
• Job Location: Gunnison, CO 81230
• Shift: Day Shift + Call (1–2 nights/week, 1–2 weekends/month)
• Duration of Contract: 13 weeks
• Guaranteed hours: 36
• Note: rural Gunnison does not have public transportation which will require the traveler to have access to a reliable source of transportation.

Submissions Requirements:

• Active Colorado RN license (Required)
• Minimum 3 years of nursing experience
• BLS/BCLS certification required
• Experience in Home Health and/or Hospice strongly preferred
• Familiarity with EPIC EMR preferred
• Must pass Colorado Adult Protective Services (CAPS) check

Job Duties in Order of Importance:

• Deliver skilled nursing care using the nursing process and individualized Plan of Care
• Conduct initial patient assessments and ongoing re-evaluations
• Develop, implement, and adjust care plans based on patient needs
• Provide preventive, rehabilitative, and palliative nursing care
• Administer treatments requiring advanced clinical skills
